Our Mission
The CHMC Foundation for Life Transitions exists to provide compassionate support, accessible resources, and dedicated chaplaincy care for individuals and families facing life’s most difficult changes. Whether grieving the loss of a loved one, navigating end-of-life care, recovering from trauma, or adapting to new realities, our mission is to ensure no one walks that journey alone.

Our Impact: Turning Compassion Into Action
Every contribution helps us comfort families, train chaplains, and bring hope to those facing loss and transition.
We provide free and subsidized grief counseling for families in crisis and distribute multilingual bereavement resources to hospitals, hospices, and funeral homes.
The Foundation invests in the next generation of chaplains by offering educational stipends, certification support, and continuing education resources.
We partner with healthcare providers, community leaders, and faith-based organizations to provide workshops and educational programs on grief, resilience, and emotional wellness.

Every day, people experience devastating loss without anyone to guide them through the pain. The CHMC Community Fund is our way of closing that gap - providing free chaplain support, grief counseling, and healing resources to individuals and families who are grieving but can't afford care.
Your donation doesn't just go into a fund. It goes to a human being:

- A nurse processing the death of a patient
- A family trying to navigate loss together
For every $50 donated, we're able to provide a full hour of chaplain support to someone in need. With $100, we can offer follow-up sessions, and at $200, you could fully sponsor someone's path toward healing.
